Medicaid, a public health program overseen by states and jointly financed by federal and state governments, provides coverage for low-income residents, seniors, children, and individuals with disabilities—making it among the largest facets of the U.S. health care system. Because taxpayer dollars fund Medicaid, changes in local Medicaid billing patterns help reveal how public health spending is distributed throughout a community.

This analysis used HCPCS codes that are specifically labeled as “COVID-19” or list “coronavirus” in billing or reference descriptions. Consequently, the reported totals only include those claims explicitly marked for COVID-19, and do not account for services connected to the pandemic that were billed differently.

For reference, Brooklyn saw the highest Medicaid spending for COVID-19 codes in New York in 2024, with virus-related Medicaid claims reaching $3,718,101.

In Commack, two providers billed Medicaid for COVID-19–categorized services in 2024. The code “COVID Specific” made up $58,263 of the amount.

The average Medicaid payment per provider in Commack for COVID-19–labeled services was $29,295, compared to a statewide average of $29,403.

COVID-19–specific Medicaid claims contributed significantly to growth in health care spending during the main pandemic period in Commack.

Payments for all other Medicaid service categories rose by $2,058,080 from 2020 to 2024, representing an increase of 48.2%.

Commack’s average annual Medicaid payments during the two years before the pandemic were $3,296,218.

According to the Centers for Medicare & Medicaid Services, Medicaid spending at the federal and state level totaled about $871.7 billion in the 2023 fiscal year, making up approximately 18% of the nation’s total health expenditures. That’s up from $613.5 billion in 2019, prior to the COVID-19 pandemic.

This change reflects growth of about 40% in a few years, mainly due to expanded Medicaid enrollment and increased service use during and after the pandemic.

Recent federal budget actions under the Trump administration introduced proposals to significantly reduce federal Medicaid funding and shift the program’s structure. The “One Big Beautiful Bill Act,” signed in 2025, is projected to cut more than $1 trillion in federal Medicaid spending over the next decade. The law adds policies such as work requirements and greater cost-sharing, which could impact coverage and funding for certain groups. These changes are expected to increase state responsibility and slow the rate of federal Medicaid growth, even as the program continues to cover tens of millions.
